Yeah, there's two bridges as soon as you get across, just set a waypoint across the other side of the border and follow it

You just travel back on your horse or carriage. There is no restriction on re-entering America.

Completed it, end-game spoilers below.

The whole last chapter, back at the ranch, is awesome. All feels worth it, all the trouble, to be back home with the family. Then it all goes horrible, what an upsetting final flourish for John Marston. I loved it, completely wrenching. Then Jack's 'stranger' mission is perfect. Great ending, great story. I'm actually feeling slightly down and upset at how saddening the game is.
